What is diabetes?Diabetes is a serious, chronic, metabolic disease that does not allow your body to use glucose (sugar) normally. The result, high blood glucose levels, causes silent damage to nerves, circulation and various organs. Over the years this damage can result in:
Why Diabetes Self-Management Education?The best diabetes outcomes occur when you work with a diabetes team. Your doctor is a very important member of your team for medical decisions, but usually only sees you one to five hours a year. You live with diabetes 8,760 hours a year … making up to three or five decisions every hour that can affect your diabetes outcomes! That makes you the team quarterback. Diabetes Self-Management Education classes act as your coach to share effective plays and strategies that will help you win the game. In addition, research has shown that these classes help save you health care dollars over the years. Who can attend the classes?Any person with diabetes may attend classes. If you are newly diagnosed, have had diabetes for years or even attended diabetes class in the past, you will benefit. For no extra cost, you may bring a supportive spouse, relative or friend. Why group classes?Our small group setting allows you to share your difficulties and successes with others that live with the same daily trials and concerns. At the same time we work with each one of you to assess your individual challenges and needs and guide you in developing a routine that fits your daily schedule and food preferences. What will I learn during class?

I now accept my condition and am dedicated to follow a healthy life.” “I learned so much and appreciated the instructors’ positive attitudes, helpfulness and time spent with us one-on-one.” Will insurance cover the cost of classes?Nearly all insurances pay for classes like this that are accredited by the American Diabetes Association. Depending upon your benefit plan, there may be an individual co-pay involved. Some insurances, and even employers, are recommending attendance as part of their wellness plans. Medicare does have some restrictions if they have already paid for a similar class in the past. How do I register?Tell the health care provider who oversees the care of your diabetes that you want to attend this class. Via Christi Clinic will fax a referral form to this provider that they will need to sign and fax back. Call 316-689-9989 or 800-876-5111 to reserve class dates. Please complete the questionnaire you will receive in the mail and bring it to the first class along with your glucose meter, if you have one. If you don’t have a meter, one will be provided at no charge to you. When and where are the classes held?In order to complete the course, you will attend two days of classes, each held from 8 am to 5 pm The class meets on the fourth floor at Via Christi Clinic on Murdock (3311 E.
