Description

Adoption assistance

After 90 days of employment

Reimbursement for adoption expenses of 80%, up to a maximum of $1,500 per adopted child.

Allstate Workplace optional insurance

First of month following 31 days of active employment

Universal life with optional long term care rider, critical illness protection, cancer protection and accident assistance plans. Offered through Star Robbins & Company.

Basic accidental death and dismemberment insurance

First of month following 31 days of active employment

Managers receive 2 times and full time employees receive 1.5 times base annual salary from Via Christi. Part time employees receive a flat $10,000 benefit.

The Medical Community Credit Union is located near Via Christi Hospital on Harry with a branch office at the West River Plaza building. The Credit Union of America has a branch in Via Christi Hospital on St. Francis and in the Medical Arts Tower.

Three dental PPO plans from which to choose. Premiums deducted on a pre-tax basis.

Dependent life insurance

First of month following 31 days of active employment

Fixed amount options of life insurance for employee's dependent children. Spouse life insurance available in $5,000 increments with $10,000 minimum purchase up to 50% of your combined optional and basic life insurance coverage, not to exceed $800,000. Any coverage amount elected above $50,000 will require a Personal Health Statement.

Compensation for scheduled work hours missed when required to report for jury duty.

Long term disability

First of month following 31 days of active employment

VCH LTD plan provides 60%. Plan provides 60% of adjusted salary after a 180 day elimination period until medical disability ends or until normal retirement age. Maximum monthly benefit = $15,000. Plan contains 2 year "Own Occupation" provision. Part time employees are not eligible.

Medical Insurance

First of month following 31 days of active employment

Choice of two comprehensive PPO medical plans which include prescription coverage. Premiums deducted on a pre-tax basis (24 pay periods per year).

Optional accidental death and dismemberment insurance

First of month following 31 days of active employment

You can purchase up to 5 times your base annual salary not to exceed $800,000 to supplement group life insurance program. Available in $5K increments with $10K minimum purchase. Any optional life insurance amount exceeding $350,000 will require evidence of insurability.

You can purchase up to 5 times your base annual salary not to exceed $800,000 to supplement group life insurance program. Available in $5K increments with $10K minimum purchase. Any optional life insurance amount exceeding $350,000 will require evidence of insurability.

Paid time off

Begin accruing immediately

Paid time off is a flexible program used to manage time away from work: vacation days, holidays, sick days or personal days. Full time non-management accrual: less than 5 years of service — 25 days per year; 5–9 years of service — 30 days per year; 10+ years of service — 35 days per year.

Pension plan

Immediately

Contributions made for years in which 1,000 hours are paid (except PRNs). Vesting credit for years in which 1,000 hours are paid (5 year cliff vesting schedule). Annual contribution based on years of vesting credit. Contributions are: less than 5 years of service — 5.5%; 5–9 years of service — 6.5%; 10+ years of service — 7.5%.

Savings plan

Opportunity to participate in a tax-deferred savings retirement program. Match provided after one year of service. Match is $0.25 on the dollar up to 4%. Three year cliff vesting schedule.

Service awards

After 5 years of service, in 5 year increments

Service awards presented at annual celebration to recognize employee's years of service with Via Christi.

Short term disability

First of month following 31 days of active employment

Via Christi provides 60% of base pay after 14 day elimination period until medical disability ends or 90 days after disability began. Maximum monthly benefit = $5,000. Short term disability is not available to part time employees.

Spending accounts

First of month following 31 days of active employment

Medical and dependent care spending accounts available to use tax-deferred money to be reimbursed for qualified, eligible expenses. Flexible spending prepaid credit card provided for medical. Most OTC medicine and drug purchases are not allowed without a physician's statement. IRS incurred extension rule "grace period" included.

Travel accident insurance

Immediately

Provides a benefit of 10 times your base annual salary to a maximum of $500,000 for loss of life, limb, sight, hearing or speech when travelling in the course of business.

Tuition reimbursement

After 90 days of employment

100% reimbursement of applicable tuition expenses up to $2,000 for full time employees and $500 for part time employees per year.

First of month following 31 days of active employment
